Patient presents with complaints of right knee pain.  Onset of pain reported as (onset date or duration).  Character of pain described as (sharp, dull, aching, throbbing, burning, etc.).  Pain severity reported as (mild, moderate, severe) or quantified on a pain scale of 0-10 as (numeric pain score).  Location of pain specified as (anterior, posterior, medial, lateral, diffuse) aspect of the right knee.  Patient identifies (exacerbating factors, e.g., weight-bearing, stairs, prolonged standing) and (alleviating factors, e.g., rest, ice, elevation).  Associated symptoms may include (stiffness, swelling, locking, clicking, popping, instability, limited range of motion, warmth, redness).  Past medical history includes (relevant prior diagnoses, surgeries, injuries related to right knee or musculoskeletal system).
