Posterior capsule opacification (PCO), also known as secondary cataract or after-cataract, was diagnosed in this patient following cataract surgery.  The patient presents with complaints of blurred vision, decreased visual acuity, glare, halos around lights, and difficulty with night driving.  Slit-lamp examination reveals thickening and opacification of the posterior lens capsule, confirming the diagnosis of PCO.  The patient's visual symptoms correlate with the observed posterior capsular changes.  The anterior chamber is quiet with no signs of inflammation or other complications.  Intraocular pressure is within normal limits.  Previous medical history is significant for cataract extraction with intraocular lens implantation.  Current medications include [list medications].  Treatment options, including Nd:YAG laser capsulotomy, were discussed with the patient.  Risks and benefits of the procedure were explained, and the patient elected to proceed with Nd:YAG laser capsulotomy.  The patient understands the post-operative instructions and follow-up schedule.
