Patient presents with complaints possibly indicative of a right adnexal cyst.  Symptoms include (but are not limited to) pelvic pain, right lower quadrant pain, abdominal discomfort, bloating, irregular menstrual cycles, and dysmenorrhea.  Differential diagnoses considered include ovarian cyst, follicular cyst, corpus luteum cyst, hemorrhagic cyst, endometrioma, paraovarian cyst, and other adnexal masses.  Physical examination revealed tenderness on palpation of the right adnexa.  Pelvic ultrasound was performed demonstrating a simple right adnexal cyst measuring [size] cm.  No complex features such as solid components, septations, or thick walls were observed.  CA-125 levels were drawn and results are pending.  Assessment: Right adnexal cyst, likely benign.  Plan: Conservative management with observation and repeat pelvic ultrasound in [timeframe] to assess for changes in size or characteristics.  Patient education provided regarding symptoms to monitor for, such as worsening pain, fever, or signs of ovarian torsion.  Patient instructed to return to clinic for follow-up evaluation and discussion of management options if symptoms persist or worsen.  ICD-10 code N83.20 (Unspecified ovarian cyst) is considered pending further evaluation.  Medical coding and billing will reflect the final diagnosis and procedures performed.
