Patient presents with complaints concerning for a right adnexal mass.  Chief complaint may include pelvic pain, abdominal distension, or abnormal vaginal bleeding.  Patient may be asymptomatic.  Relevant history includes menstrual history, last menstrual period (LMP), gravidity, parity, history of ovarian cysts, endometriosis, pelvic inflammatory disease (PID), or previous abdominal surgeries.  Family history of ovarian cancer or other gynecological malignancies was reviewed.  Physical examination reveals tenderness or a palpable mass in the right adnexal region on pelvic examination.  Pelvic ultrasound was performed demonstrating a right adnexal mass measuring [size] cm with [description of characteristics - e.g., simple or complex, cystic or solid, presence of septations or solid components].  Differential diagnosis includes ovarian cyst, ectopic pregnancy, tubo-ovarian abscess, hydrosalpinx, and ovarian neoplasm.  Preoperative laboratory tests including a complete blood count (CBC), comprehensive metabolic panel (CMP), and CA-125 were ordered.  Patient was counseled on the findings and treatment options including expectant management with repeat ultrasound, medical management, or surgical intervention such as laparoscopy or laparotomy.
