Thank you for attending the neurology clinic. I am writing to summarize aspects from your recent consultation. You present with recurrent headaches characterized by visual disturbances, nausea, and sensitivity to light and sound. These symptoms have been occurring approximately twice a month for the past year.  
You have not experienced any weakness, numbness, or speech difficulties associated with your headaches.  
Your mother also suffered from migraines, which suggests a possible familial link.  
You live alone, do not smoke, and consume alcohol occasionally. You are currently not driving due to the frequency of your migraines.  
Previous investigations, including a CT scan, showed no abnormalities.  
The neurological examination revealed no focal neurological deficits, and your reflexes were normal.  
Summary & Differential Diagnosis:  
The primary diagnosis is migraine with aura, but we will conduct an MRI to exclude other potential neurological disorders.  
Management Plan:  
1. Continue with Sumatriptan for acute migraine attacks  
2. Initiate a daily preventive medication, such as Topiramate, to reduce migraine frequency  
3. Schedule follow-up in three months to assess treatment efficacy
