Effectiveness:  
Patient notes a significant reduction in anxiety symptoms but still experiences occasional panic attacks.  
Concerns and Issues:  
Patient is concerned about the nausea and dizziness and is worried it might affect his work performance.  
Assessment of Medication Regimen:  
- Current medications appear appropriate for managing the patient's anxiety and panic disorder.  
- Reported side effects of nausea and dizziness are noted and may impact the patient's daily functioning.  
- Medications are effectively reducing anxiety symptoms, though occasional panic attacks persist.  
- Adherence issues with Lorazepam noted, possibly due to forgetfulness.  
Plan:  
- Adjustments to Medication: Consider reducing Sertraline dosage to 25mg to alleviate side effects. Continue Lorazepam as needed.  
- Additional Testing: No further tests required at this time.  
- Patient Education: Advised patient on the importance of consistent medication adherence and strategies to remember Lorazepam. Discussed managing side effects and when to seek medical advice.  
- Follow-Up Plan: Schedule follow-up appointment in 4 weeks to review medication effectiveness and side effects.
