A continuous, 24-hour form of renal replacement therapy used to treat acute kidney injury (AKI) and fluid overload in critically ill patients who are hemodynamically unstable and cannot tolerate intermittent hemodialysis.
Indicated for patients with severe AKI, fluid overload unresponsive to diuretics, and/or hemodynamic instability who cannot tolerate conventional hemodialysis. Different modalities of CRRT exist, including continuous venovenous hemofiltration (CVVH), continuous venovenous hemodialysis (CVVHD), and continuous venovenous hemodiafiltration (CVVHDF).
"Plan: Initiate CRRT (CVVH) due to worsening AKI and fluid overload, refractory to medical management."
Used to document the decision to initiate or continue CRRT in a critically ill patient. It's important to specify the modality of CRRT being used.
