1 - Proposed service (350 words max)  
Our proposed service is designed to deliver comprehensive gynecological care to homeless and multicultural communities in the Northern Sydney area. The service model will incorporate mobile clinics and community health centers to ensure accessibility. The target demographic includes women from various cultural backgrounds and those experiencing homelessness, who often encounter significant health disparities. The service will be customized through culturally sensitive methods, including multilingual support and community health workers. We will implement the service as a Syndicate, collaborating with local health providers. The service will align with the Quintuple aim by enhancing health outcomes, improving patient experience, reducing costs, boosting provider satisfaction, and promoting health equity. Services will be provided throughout the entire SNHN region, concentrating on specific LGAs with higher needs.
2 - Organisational capability and capacity (250 words max)  
Our organization has extensive experience in delivering gynecological services to diverse populations, including the homeless and multicultural communities. We have been operating in the Northern Sydney Region for over a decade, aligning with the SNHN vision of community collaboration for improved health outcomes. Our workforce comprises experienced gynecologists, nurses, and support staff, with ongoing training programs to ensure high-quality care. We are prepared to recruit additional staff as necessary to meet the demands of this service.
3 - Culturally Safe Services (250 words max)  
We have a strong history of providing culturally appropriate and safe services. Our staff undergo regular cultural competency training, and we have established partnerships with local cultural organizations to ensure our services meet the needs of diverse communities. We offer multilingual support and employ community health workers from various cultural backgrounds to facilitate trust and communication.
4 - Collaboration & Integration (250 words max)  
We will actively engage with General Practitioners, Allied Health professionals, and social care services, including housing and education providers, to ensure an integrated approach. Our experience in stakeholder engagement includes involving consumers in service design and delivery. We will adopt a multi-disciplinary and holistic approach to care, ensuring comprehensive support for our target communities.
5 - Clinical Governance & Quality Assurance (250 words max)  
Our organization has strong quality systems in place, including clinical governance, safety, and risk management frameworks. We are committed to continuous quality improvement, with strategies to implement accelerated improvements as needed. Our quality assurance processes ensure that we maintain high standards of care throughout the service lifespan.
6 - Value for Money (250 words max)  
We request funding of $500,000 (GST exclusive) for 12 months (October 2024 - October 2025). The funding breakdown includes $300,000 for staffing, $100,000 for mobile clinic operations, and $100,000 for community health center support and multilingual services.
6 - Environmental Sustainability (250 words max)  
Our organization is committed to reducing our carbon footprint. We have implemented measures to reduce emissions from transport, including promoting telehealth services and optimizing staff commutes. Our proposed service will minimize energy use through efficient practices and reduce waste by implementing recycling systems and using recycled products. We align with SNHN's Climate and Health Strategy, embedding sustainability in our core functions.
